Coming home from hospital can happen faster than expected, sometimes leaving little time to prepare. Hospital discharge care is about having someone in place to help you settle back in safely — support with personal care, mobility, meals, medication prompts in line with your discharge paperwork, and a familiar, reassuring presence while you regain your strength. We can also help you and your family understand what was discussed at discharge and what to keep an eye on, working alongside whatever support your GP, district nurse or hospital team has already put in place. Any clinical or nursing tasks, such as wound care, remain the responsibility of the appropriate healthcare professional — our carers focus on the everyday, practical side of recovering at home.

How assessment and arranging care works
Where possible, get in touch as soon as discharge is mentioned — even a day or two of notice makes it easier for us to arrange the right support in time. We will talk through the discharge plan with you and carry out an assessment, and where it is helpful and with your consent, we can also speak to the ward or discharge team to understand what has already been arranged. From there we match a carer to your situation. If discharge happens at short notice, we will do what we reasonably can, but timing always depends on the specific need and carer availability at that moment.
